Hammill and used by Ian Richardson (Wildcat Engineering), they are 
manufactured by the Los Angeles Sleeve Co., Inc., 12051 Rivera Road, 
Santa Fe Springs, CA 90670.  Email: info at lasleeve.com  They are 
available from the manufacturer at US$79 per hole plus shipping and he 
is selling them as fast as he can make them.  That's $632 plus machine 
shop charges.  How bad do you want a bulletproof crossbolt block?
